Development of a Primary Care Autonomy System

The Supreme Council of Health (SCH) decided to develop a comprehensive family medicine-based primary care autonomy system to improve effectiveness of services while ensuring financial sustainability at the same time. For this purpose, Enera was contracted to provide strategic advisory services towards establishing an integrated, cost-effective and patient-centered primary care system.

Through extensive fieldwork, we first identified ongoing problems, root causes and improvement areas. Then, in close collaboration with the leadership of the SCH, we determined strategic objectives for the primary care reform in Bahrain, which encompassed optimizing resource allocation, empowering healthcare providers, strengthening continuity of care, improving prevention, protection, and promotion activities, and enhancing financial sustainability.

In order to achieve these strategic objectives, we developed a strategic plan and a business case, designed a new governance structure, and formulated payment mechanisms and remuneration models. We created a Target Operating Model for the establishment of the Primary Care Autonomy Body, as well as operational models for urgent care and laboratory services.

Finally, we developed a comprehensive integration and implementation plan to ensure a seamless transformation of the primary care system.

Impact of the Project

The project highlighted existing issues and improvement areas in the primary care system and provided a comprehensive plan for the reform.  The Supreme Council of Health initiated transformation processes based on the strategic plan with piloting certain elements of the provided business case.
